as far as your issue I need a little bit of information:
PS3 CFW version?
Are you using a debug eboot?
Game Version?
Was it downloaded or do you have the physical disc?
Are you running it from your HDD or Network?

Was it downloaded or do you have the physical disc? : Downloaded
Are you running it from your HDD or Network? : Internal HDD

Alright, so I have 2 possible fixes for you 🙂

1 : go to Game Data on your PS3 and erase your current Data for MGSV, then re-download the update and try that.
2 : Re-Download the entire game, or get a physical copy of the game and play from that

I'm thinking something got written incorrectly to your dev_hdd0/game/ folder
